It may be time to schedule your first screening colonoscopy, a vital tool in early detection and prevention of colorectal cancer, one of the most common and potentially deadly types of cancer.

WHAT IS A SCREENING COLONOSCOPY?

A screening colonoscopy is a medical procedure to detect and prevent colorectal (colon) cancer. It is also an effective tool to detect and remove colonic polyps and stratify the patient’s risk factors for developing colon cancer.

Performed by a board-certified gastroenterologist, a thin, flexible tube called a colonoscope is inserted under sedation to examine the inside of the colon and rectum. The colonoscope has a small camera attached which allows us to visualize the lining of the colon and identify any polyps, growth, or inflammation that could eventually develop into cancer over time.

If any polyps or abnormal tissue is discovered during the procedure, we can often remove it immediately. The removal of polyps is a proactive step in the prevention of the development of colorectal cancer.

The procedure usually requires a prep including a clear liquid diet and a laxative preparation to clean out the colon. The colonoscopy is performed under sedation, so you should not feel any discomfort or pain, but you will need someone to drive you home.

ARE THERE ALTERNATIVES TO A SCREENING COLONOSCOPY?

A virtual colonoscopy, sigmoidoscopy, a fecal immunochemical test, a fecal occult blood test, or a stool DNA test are all possible options. However, exploring alternatives is a conversation we choose to have with our patients. There are many factors to consider including age, family history, and previous results. As gastroenterologists, we prefer not to generalize and look forward to scheduling a time to chat with you in our office.

AT WHAT AGE DO YOU NEED A SCREENING COLONOSCOPY?

According to the most recent studies, if you are not at a high risk for colon cancer, you should schedule your first colonoscopy at age 45. If no polyps are found, you probably won’t need another colonoscopy for another 10 years. But if you have a family history of colon cancer, a personal history of polyps, inflammatory bowel disease or other medical issues, you may need to schedule more frequent screenings and perhaps even at a younger age.
In general, regular colonoscopy screenings are recommended from age 45 to 75. While officially, there is no upper age limit for colorectal cancer screening, most US medical organizations believe the benefits of colonoscopy decrease after age 75. There is minimal evidence to support this screening procedure after age 85.

WHAT ARE THE COMMON RISK FACTORS FOR COLON CANCER?

Are you an African-American or an Ashkenazi Jew? Do you have a history of ulcerative colitis or Crohn’s disease? Does colon cancer run in your first-degree relatives? You may be at a higher risk of colon cancer.

Here are other common risk factors:

  • Age: Your risk increases with age, particularly after age 50.
  • Genetics: Certain inherited genetic syndromes, such as Lynch syndrome, can elevate your risk.
  • Lifestyle factors: Obesity, smoking, and heavy alcohol consumption are associated with a higher risk of colon cancer.
  • Diet and exercise: A poor diet and lack of physical activity are associated with an increased risk of colorectal cancer.
  • Radiation exposure: High levels of exposure from cancer treatments can be a risk factor.
  • Medical history: A personal history of colonic polyps may increase your risk for colon cancer.
    While these factors may contribute to a higher risk for colon cancer, this disease can develop in individuals with no apparent risk factors. Regular screenings and a healthy diet are key proactive measures.
